Frimley Trainers Group

Frimley Trainers Group represents the GP Trainers who work in the practices around Frimley Park Hospital,  within the KSS Deanery area. We are an active group of 20-25 Trainers working in about 15 practices. We hold a variety of meetings - residential workshops, lunchtime meetings, evening meetings. The group is currently co-ordinated by Dr Richard de Ferrars.

This section of the website contains information and ideas that we have accumulated in recent years. As you can gather, we are always open to ideas and suggestions! There is a combination of material for the 2007 changes to the MRCGP and also material reflecting "older" aspects of GPR training

GP Registrar Starting Information

There are continuing revisions to all aspects of starting with a GPR/ ST3. The first section of the GP Training Handbook, available on the KSS website, explains all of this is in detail. KSS Website - Downloads holds most of these documents

Pre-Arrival

GPR needs to: Visit the KSS website and download the GPStR Training Handbook
  GPStR Payment Form replaces the FPGPR1 form for salaries. Should be sent to ST3 by the Deanery and requires trainer signature 
  Complete the Medical Performers List Application - contact the relevant PCT who will the forms directly to the GPStR
  Register with RCGP (unless registered in ST1 or ST2)
  Sort out MPS / MDU
   
Trainer needs to: Check GMC Registration on GMC website
  Send practice information - eg Practice Booklet / website details
  Sign off the GPStR Payment Form
  Sign employment and educational contracts (KSS Website - Downloads)
  Complete GPST SLA (KSS Website - Downloads)
  Ensure they understand the Car Allowance regulations (KSS Website - Downloads)
  Consider sending educational preparation material, eg Knowledge Checklist, Background reading

Introductory Phase

In the future, GPRs/ ST3s will increasingly have spent time in the practice in ST1 and ST2. This can be used for some of the traditional "introductory programme" tasks including basic computer training and joining other PHCT members. We are hoping that this will allow a streamline introductory phase with own surgeries starting in week 3 (week 1 will start on a Wednesday). The main tasks in this phase are:
         ·     Getting to know the doctors - sitting in at least once with everyone
         ·     Computer training
         ·     Learning how the practice functions

These computer training session worksheets are specific to the way that we use EMIS at FGMC but they can be adapted for use at your own surgery. They are designed to fit with "Using EMIS" and generate a "hands-free" way of getting FY2's and ST's up to speed with EMIS


Getting Started with MRCGP

Check your GPR is registered with the RCGP (we do advise them to become Associates in Training)
Check that they have received details for their e-Portfolio
Talk through the year-planner at an early stage. In particular, discuss:
            - planned AKT dates
            - planned CSA dates (affects speed of progression onto 10 minute appts.)
            - OOH training

Formative Assessment Information & Tools

We have recently updated two old tools to make them useful with the updated MRCGP. They are part of the "Formative Assessment Workbook" that can be downloaded (link at the end of this section).

Knowledge Checklist This updated checklist is derived from the new MRCGP Curriculum. Each section has a "Learning Resources" section to allow the trainee to explore educational in areas with low scores. (Similar concept to Curriculum Map on RCGP website)
Competence Checklist
This is a slightly modified version of the MRCGP "Domains of Competence"
It includes a couple of additional descriptors that used to be in the MARS
It includes a comments and checklist facility that enables the checklist to be updated through the year in the way that the Trainers Report was used
In summary - merges MARS, Trainers Report with Domains of Competence

Under the old Summative Assessment scheme, most trainers locally were operating a 3-4 month review cycle. Usually these would happen in month 2, 5-6 and 8-9. Each review would consist of:
    - Knowledge check using a checklist, PEP or confidence rating scale
    - MARS review
    - Review of GPR's learning log
    - Review of progress in "required tasks" - SA, MRCGP, OOH, minor ops etc
   
- Agreeing a plan for use of tutorial time in the next 3-4 months

Under the 2007 revised MRCGP, there is a requirement for a "nodal" review at month 6 of the year and the final review at month 10. These reviews duplicate a lot of the content of the previous review cycle but first review at month 6 is too late for planning activity. By introducing an introductory meeting in month 1 and a formal review in month 2, the old cycle continues and encompasses the WPBA & e-portfolio reviews required in months 6 and 10. The Formative Assessment Workbook includes updated templates for these reviews.

Personal Development Plans

These are some of the standard forms used in PDP folders:

Trainers are required to submit their "Trainer's PDP" to claim the additional allowance. These were some suggestions on generic content: Trainer's PDP - suggestions

 

ST1 & ST2 Study Leave Attachments

As a group, we are recommending 2 days per 4 month post, with a fee of £100 per day. If the trainee has budget restrictions, then 1 day per post is an acceptable minimum. If a trainee does not wish to engage even for 1 day per post, this is going to cause problems with the reviews for the WPBA - please contact the relevant Course Organiser / Programme Director. A payment of £200 from the study-leave budget for the 2 days roughly equates to a session of GP time.
Time spent with other PHCT members should be relevant and focused. Time spent with trainer should focus on communication skills, the consultation and clinical issues relevant to current post.

What should be organised to provide a good experience?
Sessions:
    
Sitting-in with trainer 
     Sitting in with a GPR and having a good chat
PHCT experience relevant to their current post:
     A&E - minor ops session, OOH session (2pm - 10pm day)
     Medicine - focus on one chronic disease with teaching session and nurse clinic
     Geriatrics - nursing home visit, time with District Nurse
     O&G - time with Midwife
     Paediatrics - time with Health Visitor
     Orthopaedics - minor ops session or collect some joint injections 
Education & Portfolio:
    
Nodal reviews in months 6-7 (post 2) and month 10 (post 3)
     Review of post PDP and learning objectives
     Review of learning log
     Review of assessments

Out of Hours Work

The ST3 still needs to complete an OOH workbook - enter "OOH" into the search box on the KSS Website. The ST3 also needs to complete entries in their e-portfolio for every OOH session. National guidance still recommends 12 OOH sessions as typical experience required to achieve competence. These OOH sessions should be 6 hours duration, although busy shorter sessions may be acceptable. The e-portfolio MUST record 12 completes sessions for CCT.

With each session, the OOH supervisor should complete a record sheet (linked below) and should share this with their ES. 

After each session, the trainee should also complete a record in their learning log / e-portfolio and link it to section 7 (Care of Acutely Ill People). You will note that competencies outlined under the "Primary Care Management" domain are identical to the headings in the OOH Workbook

At the end of the training programme, the educational supervisor will search for all OOH sessions in the ‘shared entries’ in the e-portfolio (there exists a filter facility for this) ensuring that the requisite number have been completed. A declaration by the educational supervisor is then completed which will appear in the ‘progress to CCT’ section of the e-portfolio
